THE PROBLEM

Around one million people visit the Emergency Department each year following a head injury. While the majority of these people will experience no lasting effects, many others will be left with a traumatic brain injury (TBI) that can have devastating and lifelong effects.

Traumatic Brain Injury (TBI) is an injury to the brain caused by a trauma to the head (head injury). There are many possible causes, including road traffic accidents, assaults, falls and accidents at home or at work.

​

Current practice and equipment limitations mean that a patient with a suspected traumatic brain injury will likely be immobilised flat onto a board. Then a choice has to be made between protecting the C-spine by limiting movement or adopting a 30 degree head up position conducive to prevent or limit brain damage. Adopting a flat position is dangerous for the following reasons:

Against guidelines – Guidelines state that a 30 degree heads up position is optimum in the early stages post injury - There is no current way to follow the guidelines.

​

Vomit – Aspiration of vomit in the supine (Flat) position leads to hypoxia.

​

Raised Intercranial Pressure (ICP) -  Reduces Venous drainage and cerebral perfusion pressure causing hypoxia to the brain.

​

Improvised, inconsistent practice, Attempts to raise the head are often improvised, it does not offer the correct incline and can be dangerous and render restraints and seatbelts useless.

​

Dangerous practice – Dangerous to the patient and clinician, standing in moving vehicles, to turn the immobilised patient on their side if vomiting. Negating any safety features should there be a crash.​​​

•It is the most common cause of death under the age of 40 years.

•Severe traumatic brain injury (TBI) is the leading cause of death in children in the UK, accounting for 15% of deaths in 1-15 year olds and 25% of deaths in 5-15 year olds.
